What is the "smallest" tractor/mower with: foot operated hydro-static, 3pt capabilities, and front/rear hydro ports?

425? Reason I ask is I'm considering stepping up from a GT235.

As far as I know, the 425 sounds like the machine you want, I don't know that Deere has a provision for rear hydraulics though. The 3 point hitch on those use the mower deck lift cylinder
If you are looking at a 425, you might want to consider the 445 which is basicly the same tractor. The reason I say this is because 445 is EFI, electronic fuel injected, which is more fuel efficient and no points to worry about.
Some Bolens large frames fit the bill, but they're fairly large by gt standards.
Yes there is a rear hydro kit for the 4x5s. Its rare to find any rear hydro outlets already installed on a tractor because it gets very pricy when you add the cost of modifying the tractor and the implement for hydro operation. Deere probably still has new kits in the parts system that fit 4x5 machines that you could buy.

If you are going for the 445 why not step up to the 455 that is a diesel? 425/carb, 445/EFI, 455/diesel

I have a 445 I bought this last April and I like it. I did not look into this model too deeply at the time to know about the carb/EFI/diesel thing or I may have looked for the 455.
Come fall I am going to install the garden plow to turn dirt over for nexts year planting.
Mine only has the front hydro for the 4 way snow plow I have. It does not have rear hydro (was it an option for this GT?) or a rear PTO that was an options for this model GT.
Mine does have a 3PH out back that the garden plow will hook to.
